How does it work?
Getting started is simple. You can call or text, fill out our referral form, or your healthcare provider may refer you. One of our Connect representatives will help determine which of our free programs and services best fit your family’s needs. Many of our families benefit from multiple services.

Who is eligible?
Pregnant women, infants and families with young children who are at-risk for poor birth outcomes, unstable home environments and/or child developmental issues. Eligibility is determined by the Connect Screen, a series of questions and answers with a Connect representative on the phone.
